About This Role

The Registered Nurse (RN) and Licensed Practical Nurse (LPN) at Gracepoint Home Care provides compassionate and proficient skilled nursing care to clients in their homes. This position is responsible for maintaining high standards of care, ensuring compliance with regulatory requirements, and maintaining clinical documentation outlined in the client's care plan.

Qualifications

  • Bachelor of Science in Nursing (BSN), Associate of Applied Science in Nursing (ADN - Registered Nurse), or Licensed Practical Nurse (LPN) license required. 

  • Valid nursing licenses in the state where skilled services are being provided. Multistate licenses are preferred. 

  • Experience in home health care or similar settings, with specific expertise in skilled nursing services Gracepoint provides preferred. 

  • Excellent communication skills, both verbal and written, and proficient in nursing documentation. 

  • Knowledge of healthcare regulations, accreditation standards, and quality assurance procedures. 

  • Valid references must be provided.  

Job Description

Client Clinical Care

  • Provides direct nursing care based on the administrative assessment and skilled nursing care plan. 

  • Follows client’s Plan of Care. 

  • Completes and documents accurate physical assessments for each shift. 

  • Demonstrates proficiency in performing nursing procedures including medication administration, wound care, treatments, and/or any other individualized interventions as dictated by the skilled nursing care plan. 

  • Thoroughly documents medication administration, provides clear and concise nursing notes, treatment notes and other pertinent documentation during the scheduled shift. 

  • Educates patients and/or family members when performing procedures or administering medications or treatments. 

  • Adheres to and follows all compliance, policies, and procedures set up by Gracepoint Home Care for skilled services. 

  • Communicates effectively with patients, families, and other healthcare team members. 

  • Provides compassionate and comprehensive care to patients. 

Compliance and Quality Assurance

  • Compliant with federal, state, and local regulations, Gracepoint polices and accreditation standards. 

  • Communicates changes in care to the Clinical Services Manager. Examples are doctor visits, hospital stays, and/or changes in medications, equipment, and treatments. 

  • Ensures all clinical documentation is up to date, thorough, and accurate in the electronic health record (EHR). 

  • Maintains up-to-date knowledge of medical practices relevant to the skilled home care setting. 

Shift Requirements

  • Clocks in and out from the client's home on time for every scheduled shift. 

  • Completes daily nursing notes, treatment records, and medication entries before clocking out for the scheduled shift. 

  • Communicates paid time off and shift requests before the 21st of the month. 

  • Maintains a professional appearance. Wears Gracepoint approved attire with logo every shift. Always wears the Gracepoint name badge. 

  • Communicates all pertinent matters to the Clinical Services Manager during the assigned shift. 

  • Adheres to ethical standards and legal regulations governing nursing practice. 

  • Collaborates effectively and per policy with client, family, and the administrative team to ensure patient safety, infection control and positive client outcomes. 

Skills

  • Proven experience as a registered nurse
  • Excellent knowledge of nursing care methods and procedures
  • Excellent knowledge of emergency care
  • In-depth knowledge of health and safety guidelines and procedures (sanitation, decontamination etc.) and willingness to follow them at all times
  • A team player with excellent communication and interpersonal skills
  • Responsible and compassionate
  • Outstanding organizational and multi-tasking skills
  • Patient with excellent problem-solving skills
  • BSc or other diploma from a nursing program
  • Valid nursing license

Job Types: Full-time, Part-time, PRN

Pay: $35.00 – $40.00 per hour
