The CNA/Caregiver at Gracepoint Home Care is responsible for providing compassionate care and assistance to individuals who need support in their daily activities due to aging, illness, disability, or other conditions. The position is vital for helping clients maintain their independence and quality of life while ensuring their safety, comfort, and well-being. Job Description

Responsibilities

• Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and personal hygiene.
• Provide mobility assistance, including transferring clients from beds to chairs, helping with walking, and ensuring safety during movement.
• Assist with feeding and meal preparation, ensuring that clients follow dietary restrictions as needed.
• Engage clients in meaningful conversations and activities to promote mental stimulation and emotional well-being.
• Accompany clients to appointments, errands, social activities, and recreational outings as needed.
• Provide emotional support, encouragement, and reassurance to clients and their families.
• Perform light housekeeping duties that clients can’t complete on their own
• Report any unusual incidents
• Act quickly and responsibly in cases of emergency

Skills

• Previous experience in caregiving or a related field is preferred.
• Knowledge of basic healthcare procedures.

• Ability to lift and transfer up to 50 pounds.
